  • Payment needs to be made monthly by ACH through your bank account or paid annually using your payment method of choice.
  • 15% discount on services when paying by cash or check (credit card and CareCredit payments are not eligible for discounted rates).
  • Declined ACH payments are subject to a $10 late fee added to your account.
  • Memberships canceled within subject to a $200 cancellation fee.
